If you meet the FACS criteria and have been assessed as needing a service will be offered a Personal Budget. You will then have the opportunity to work with people who know you to put together a Support Plan.
The Ongoing Intervention Team
The team support adults with disabilities aged 18 plus as well as parents and carers and is based at Sandringham House, 170a Overdale road, Park End, Middlesbrough, TS3 7EA.
The Transition Team support young people with disabilities aged 14 up to 25 years of age plus parents and carers and is based at Sandringham House, 170a Overdale Road, Park End, Middlesbrough, TS3 7EA.
Forensic Learning Disability Team
The integrated Forensic Learning Disability Team support adults and young people with learning disabilities who have a forensic history.
The team is based at Roseberry Park, Marton Road, Middlesbrough, contact: 01642 837447.
An 8 bed short break service and 8 bed residential care facility for people with a learning disability aged 18 to 64. Eligibility to this service will be for those who meet the FACS criteria and have an assessment of need. For further details contact 01642 727940.
Middlesbrough Friendship Group (formerly Learning Disabilities Open Partnership Board)
An open forum for people who want to make the lives of people with learning disabilities as meaningful and ordinary as every citizen of Middlesbrough. The group meets every 3 months and looks at issues which affect people with learning disabilities.
Meetings take place at the Orchard, off Lansdowne Road from 5-7pm. For further details contact 01642 728486.
