Hospital to home support

When people no longer need hospital care, being at home is the best place for them to continue their recovery. A successful hospital discharge is all about providing the right care, in the right place, at the right time.

Our Prevention Liaison Officer works with hospitals to support discharges, as well as keeping in touch with patients when they return home. They can make referrals for assistive technology, and the Connect and Telecare monitoring services.

The Prevention Liaison Officer is also a qualified 'trusted assessor'. They can refer you for handyperson services from the Staying Put Agency, like installing key safes, grab rails, and minor adaptations. They can also refer you to other services in adult social care and the voluntary sector.

The hospital to home service offers:

  • practical advice and support
  • minor adaptations
  • handyperson service
  • assistive technology or telecare equipment to help with independence
  • disabled adaptations
  • key safe installation
  • moving small items of furniture
  • checking and setting up heating
  • arranging free fire brigade safety checks
  • decluttering or emptying the fridge
  • picking up clothes for the patient to go home
  • picking up a wheelchair for the patient
  • weekly reviews by phone to track the patient's progress
