All Local Safeguarding Children Boards (LSCBs) in England and Wales are required to have in place a rapid response process (RRP) and a Child Death Overview Panel (CDOP), as outlined in Chapter 5 of Working Together to Safeguard Children.
LSCBs are responsible for collecting and analysing information about all deaths of children and young people (up to the age of 18 years) in their area. This information will identify patterns and trends, enabling specific action which may prevent deaths in the future.
The Tees Child Death Review Project (managed by Redcar & Cleveland SCB in partnership with Hartlepool SCB, Middlesbrough SCB, and Stockton On Tees SCB) provides a dedicated service to ensure that statutory requirements relating to child death reviews are implemented.
