Your Personal Adviser

Your Personal Adviser is there to help you on your journey towards becoming an adult. They can support you right up until you're 25.

Your Personal Adviser will be in charge of reviewing (looking at) your Pathway Plan, which has your plans for the future and what help you need to make them happen. The review will happen every 6 months, and is to make sure you're on track, and see if you need any more help.

The help you can get after you turn 21 will depend on your situation and what you need.

If you decide to leave the Pathways service when you're 21 but you need support in the future, you can come back to us (until you're 25), and we'll work out what help you need.

We'll try to make sure you keep the same Personal Adviser but sometimes that's not possible.
